Transition Services
Ideally, transition planning begins at age 14 with ongoing educational assessments and identification of the transitional needs on the IEP. A well-coordinated plan is essential to the students success, whether they are venturing into the world of employment, day services or college.
Our Director of Transition Services, Diane Woodward, can provide you with the following, or work with you to determine your needs or those of your students.

- Inclusive Student Transition Portfolio
- Level One, Two and Three Vocational Evaluations (Including Comprehensive Diagnostic Vocational Evaluations)
- Job Development, Job Shadowing, and Job Coaching
- Situational and Alternative Assessments
- Mobility Assessment and Training
- Long or Short Transition Consultation Services which may include:
- Tours of adult programs with students and families
- Education and assistance navigating Federal and State benefit programs
- Applying for eligibility determination for adult services
- Referral services
- Staff In-services
- Emergency Transition Services
- Services for at-risk youth
