Transition Services

Ideally, transition planning begins at age 14 with ongoing educational assessments and identification of the transitional needs on the IEP. A well-coordinated plan is essential to the student’s success, whether they are venturing into the world of employment, day services or college.

Tours of adult programs with students and families
Education and assistance navigating Federal and State benefit programs
