Summary: Facilitates the growth and community interaction of an individual with neurological impairments, learning disabilities, or autism. Provides a variety of services necessary to enable the individual to live successfully in the community while providing relief to individuals who provide primary care and support.

In-Home Residential Habilitation Worker (Permanent, PT)

Washington County Opening:
GREENWICH- A 28 year old man with multiple developmental disabilities including Severe mental Retardation, Autism and Tubular Sclerosis is looking for an experiences, permanent worker 3 to 4 times each week for 4.25 hours each visit. Must be able to lift and transport a wheelchair and to provide support and assistance to access his community for social and leisure activities such as visiting friends, watching local school sports events; going to the mall, the video store and the grocery store. Also to occasionally assist with some personal care, such as bathing.

Saratoga County Openings:
SARATOGA SPRINGS-A 13-year-old boy with Autism who lives in Saratoga Springs with his family and attends Wildwood School is looking for an In Home Worker 2 times a week for 4 hours each visit. He wants support in planning and participating in age appropriate activities in the community, making purchases with confidence, and participating in sports activities such as Challenger or SAFE. He wants support and assistance in developing conversation skills to include detail and spontaneity. He wants opportunities to discuss options and to engage in the decision making process and to explore new and unfamiliar activities and places in his city. He loves the library, computer and movies.

WATERFORD- A 30 year old woman with a Developmental Disability who attends Wildwood Day Hab and lives with her parents is looking for support and assistance 2 times each week for 4 hours each visit. She wants to be able to safely and appropriately access her community for interests such as shopping, exercise at the Cohoes Community Center, walking, attending music, recreational and social events. She enjoys watching wrestling events, baseball and playing basketball. She also wants support in increasing her independence at home in the areas of cooking and cleaning.

BALLSTON SPA- A 19 year old WWS student with a Developmental Disability who lives in Ballston Spa is seeking an In Home Res Hab worker once weekly for 4 hours to support him in his community. His interests include the outdoors, music, photography, eating out. He has a high level of energy.

BALLSTON SPA-A 13 year old boy who lives with his family in Ballston Spa is looking for an In Home Worker 2 times a week for 3.75 hours each visit. He wants opportunities to learn and practice social and safety skills in the community and at home. He is friendly, outgoing, and active. He needs age appropriate opportunities to build his self-esteem, and support to calm, focus and attend to task. He enjoys crafts, cooking, bowling, parks, swimming, going to the library, and loves to help whenever he can.

SARATOGA SPRINGS- A 43 y/o woman with a Developmental Disability who lives independently in the City of Saratoga Springs is looking for a Res Hab worker to support her in a variety of activities in and around her community. She enjoys shopping; going to the State Park; and seasonal outdoor activities such as apple picking, yard sales and fairs. She wants a worker once each week for 3.5 hours during the daytime.

BALLSTON SPA-Part-time w/ Benefits! (Mon-Fri, 3-7:15pm) to work one-to-one with a 32-year-old woman with a developmental disability who needs assistance with personal and home care tasks as well as participating in activities she enjoys in and around her community. She has extremely limited verbal skills but communicates effectively through sign and an assistive device. Training support and supervision are provided. High School diploma/GED with one to three months&Mac226; minimum experience required. Valid NYS driver's license and access to a vehicle are also required.

BALLSTON LAKE- An 11 year old boy who attends Wildwood School and has a Developmental Disability is looking for a permanent In Home Worker 2 times each week, for 3 hours each visit. He is very physically active and enjoys being outdoors, going to the local Y, music, reading and anything associated with Thomas the Tank Engine. He wants assistance and support to safely integrate into community activities such as going to the store and making purchases, using the Y, going to amusement parks, increasing his ability to communicate his wants and needs. He has some verbal speech, sign language skills and an assistive devise for communication.

Schenectady County Openings:

GLENVILLE-A 26-year-old man who lives with his family in Glenville is looking for an In Home Worker 2 times a month for 3.5 hours each visit. Due to the complexity of his medical conditions, work will take place primarily in his home. He has a nurse or family member with him at all times and no medical care is required of the In Home Worker. He wants someone to support and assist him in his newest venture of the creation of a baseball stadium trivia book. He also wants assistance in accessing his computer to expand his talents and interest in art and computer graphics, organizing his extensive collection of sports cards, & accessing media to expand his knowledge of sports, music, car & motorcycle activities of interest to him.

Albany County Openings:

DELMAR- An 18 year old male who lives with his mother needs res hab approximately 3 times per week for 4 hours per visit. He is working toward greater independence and increased recreational activities. He enjoys bowling, dining out, movies, drawing, and the arcade. Support in advocating for himself and with scheduling and money transactions.

GUILDERLAND- A 25 year old male who lives with his family in the Guilderland area of Schenectady is looking for an In Home Res Hab worker 2 times per week for 4 hours each visit. He has many interests including photography, cinematography, working out at his local Y, sports, movies. He wants assistance in decision-making, assertiveness, increased independence both at home and in his community.

ALBANY- A 12 year old boy with a Developmental Disability who lives with his mother in Albany is seeking an In Home Res hab worker once each week for 4 hours. He wants support to become more independent both at home and in his community; dressing, money skills, street crossing, negotiating crowds. Her enjoys soccer, bowling, baseball, Great Escape, the library.

ALBANY-A 22 year old woman with multiple & complex Neurological Impairments who lives with her mother in Albany and attends a Wildwood Day Services Program is looking for an In Home Worker 2 times a week for 4.25 hours each visit. She wants assistance and support to become as independent as possible in both her home and community. She enjoys music, video games, sports, swimming, bike riding, going to the Y, & being with people. She wants support to access these activities successfully. She wants support with cooking and household chores. She is learning to use her new Dynamite to expand her communications skills and wants support in using this devise.

Rensselear County Openings:

NASSAU- A 28 y/o man with a Developmental Disability who lives with his family in Nassau is looking for a Res hab worker 3 times per week for 4 hours each time. He wants support primarily in the community to learn social skills that will promote his ability to develop friendships. He enjoys a wide variety of activities and is open to trying new activities. He wants to go to age appropriate social and recreational settings such as clubs, dances, museums, plays, and get-togethers. He also wants support in developing skills for relaxation and use down time, money management, time management .

CASTLETON-A 10-year-old girl with a Developmental Disability who lives in Castleton and attends Wildwood School is looking for a Res Hab worker 2 times a week for 4 hours a visit. She has limited verbal and safety skills and needs someone who is energetic and patient. Through her love for play, music, reading, shopping, going for walks, and animals, she will increase her verbal language and communication skills, pay attention, follow simple directions, interact with those around her and be able to integrate safely into her community. The Res Hab services will take place in both her home and in her community.

JOB DESCRIPTION

Job Title: In-Home Service Residential Habilitation Worker
Reports To: Assistant Director and/or Coordinator for Residential Habilitation
FLSA Status: Non-Exempt
Department: Family and Community Support
Location: Capital District

Summary: Facilitates the growth and community interaction of an individual with neurological impairments, learning disabilities, or autism. Provides a variety of services necessary to enable the individual to live successfully in the community while providing relief to individuals who provide primary care and support.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

* Works one-on-one in the consumer’s home and community to address a variety of life skills to include social, recreational and other activities of daily living.
* Follows the goals and/or activities specified in the individual's "Individualized Service Plan" and Residential Habilitation Plan.
* In cooperation with the individual and family, the support staff helps to identify skills and competencies to be developed and devises plans to achieve those skills and competencies.
* Assists consumers to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access.
* Measures the likelihood of social isolation and develops plans to build social and communal relationships, which prevent or minimize that isolation.
* Negotiates schedules and daily activities.
* Regularly maintains and submits service notes with documentation of the hours worked and the service provided.
* Maintains and submits travel vouchers and reimbursement requests.
* Follows any agreed upon action plan for activities with the individual, including a behavior plan.
* Employs knowledge of emergency procedures as outlined by Wildwood.
* Enhances and promotes social interaction and community participation.
* Presents as a positive role model.
* Works those hours authorized by the Individualized Service Plan (ISP).
* Submits to and reviews reports and problems with the Assistant Director for Residential Habilitation.
* Provides transportation for the consumer to and from community activities.

Qualifications:

Education/Experience:
High school diploma or general education degree (GED); and one to three months related experience and/or training; or equivalent combination of education and experience.

Language Ability:
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to present effectively information in one-on-one and small group situations to consumers, families and other employees of the organization.

Math Ability:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

Reasoning Ability:
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Supervisory Responsibilities:
This job has no supervisory responsibilities.

While performing the duties of this job the employee is occasionally exposed to multiple work environments varying in exposures to heat, cold and outside weather conditions. Extensive traveling in a car is required. The noise level in the work environment is usually moderate.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand, walk and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. May occasionally need to assist in loading wheelchairs in and out of vehicles.

Additional Requirements:
Daily access to an automobile is required. A valid New York State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required. CPR and First Aid certification or eligibility for certification.
